The Band

FORM is a rock band from Itasca, Illinois, mixing original songs with classic and modern rock covers.

Formed in 2018 by Alec Tau and Jonny DiSilvestro, the band released “Otherside” and “Million Miles” before writing their debut album, E Fifth Ave. Their set pulls from decades of rock without staying in one lane — originals, familiar covers, and live energy built for the room.
